全民健保保費預收之可行方案研究

健保保費收繳,主要按月繳納,而欠費亦採按月催收。由於每月重覆作業徒增行政成本,若能簡化收繳方式,必可降低成本、提高收繳率。因此,本文擬就欠費情形及預收的可行方案進行研究。結果發現就筆數言,第二、三類具最高的欠費比率;在金額部分,則以第六類欠費比率最高。至於影響欠費及保費預收的因素,教育程度愈高或家庭所得愈高者,欠費的可能性較低。男性或年紀輕者,則明顯增加欠費的機率。而教育程度愈高者,支持保費預收的機率愈高。此外,第二、三類支持預收的機率都顯著地高於第六類。而且在預收享有優惠下,其對政策支持度,更分別高達86.0%及85.5%;第六類亦由40.5%上升到60.1%;因此整體支持度為68.2%。綜言之,保費預收具高度可行性,但在政策執行前,應妥善解決潛在問題,以免適得其反。

The Feasibility of Pre-Collecting National Health Insurance Premium

Under the National Health Insurance Act, the insured pay the premium monthly except for those in Categories 2 and 3. To deal with overdue charge and legal action caused by overdue premiums, the BNHI has additional numerous administrative costs for each month. The purpose of this paper is to examine the factors affecting the arrearage as well as to assess whether the BNHI directly pre-collecting premium is feasible. The results show that the probability of arrearage would increase if the insured were male or young but decrease with advancing education or family income. For the feasibility of the pre-collection of premium, if the BNHI goes with some benefits, the total approval rate reaches to 68.2%. In general, to implement the policy successfully, the BNHI needs to overcome some difficulties as well.
